Lead with the visit time and completed care. Then record appetite, water, toilet or litter, activity, behavior, and any medication confirmation using neutral language. Put questions and the next visit at the end so the owner knows whether a reply is needed.

Compare with the supplied normal

Caregivers should report what they observed, not diagnose it. If the owner said a cat normally hides for the first ten minutes, record whether that happened. If something falls outside the agreed pattern, follow the existing contact or emergency plan instead of guessing.

Keep medication wording factual

Record whether the supplied instruction was completed, refused, delayed, or uncertain. Do not invent a replacement dose or missed-dose response. Contact the owner, prescribing clinic, or agreed emergency contact when the written plan does not cover what happened.

Share the smallest useful record
