A regular clinic may be closed when an incident happens. Include the nearest appropriate after-hours clinic, verify its phone number, and keep a backup person who can make decisions if the owner cannot be reached.
Record facts, not diagnosis
List known conditions, allergies, current medications, identifying details, and where the carrier or lead is stored. The card is a reference for the caregiver and clinic; it cannot determine whether a symptom is urgent.
